Database Clients Arkansas Cancer Research Clinic Designed a database to keep with with their mailing lists, volunteers, and special events coordinators. By providing a comprehensive mailing list, they can assign people to various groups without having to type them in again. They have another list of merchants that sell tickets for Partner's Card, various Chefs for their Cook's Tour, and other event participants. Labels, lists, and mail merges are output easily from the Report Menus. Administrative Office of the Courts Attorney Ad Litem Program Designed a database to track case dockets concerning at risk juveniles that were represented by attorneys in the Ad Litem program. Each docket could involved multiple children who could then have multiple placements. Legal histories of each docket were added, as were invoice tracking and monthly reports from each ad Litem attorney. Parent Counsel This database tracks ad litem attorneys and cases for attorneys that represent the parent or guardian who have at risk juveniles. Designed a price change database with easy point and click controls. The database is used to notify vendors of special deals that AF has going on. Reports include price change history as well as products list by vendor. AireTech Corporation Created a database to track bids to contractors. Also a PO and Invoicing system. ALCOA - Industrial Chemicals Division Created a time keeping system based on employees hours spent on certain projects. Reports included YTD graphics, vacation time used, summary reports by major category and projects. The system was designed to be very easy to use with a variety of push button controls. I also created a complaint tracking system to log in customer complaints about products or service or other areas. Reports can be accessed for any period of time by simply typing in a range of dates and selecting the type of report you want. Data can also be exported to Word or Excel with a click of a button. Another database was developed for the concrete division for testing samples was created from spreadsheet data which greatly simplified their monthly report process. A recent database tracks safety issues and good safety practices. ALLTEL Information Systems Created a project tracking system to keep up with time spent on projects. Reports could be generated for any time period with simple fill in the blank menus. I also created a job logging system for Alltel’s Copy and Print Services in Maumelle to keep up with print jobs like business cards, forms, letterhead, etc. The system generates reports automatically by prompting the user to enter dates, etc. There have been several other databases I’ve worked on for ALLTEL other than the two above over the past few years – some which were developed by others originally and redesigned by myself. American Legion Auxiliary Worked with staff on an existing database to group the data easily and to avoid repeating fields in multiple tables. American Scientific Testing Created a telemarketing database for underground tank testing. Screens included push button controls to bring up the owners of the tanks as well as information about the tanks themselves so the telemarketing manager would have all the information in front of him when he made the calls. Reports included fill in the blank forms so the tank tester could just print out a completed report. Arkansas Bar Association Created a complete member database; all with point and click ease, from an existing R-Base file. The ABA also keeps up with several non-member associations with the need to run labels and lists in various ways. Each type of association has their own menu screen and reports. A second database was created to keep up with continuing legal education credits and seminars offered by the ABA. This database was tied to the membership database to avoid duplication of effort. Arkansas Department of Corrections Worked with staff to create database to track expenses. Arkansas Cardiovascular Surgical Associates Created a patient surgical history database with both routine and customized reports. The database provides a simple tracking method for both the doctor’s and the surgical staff to use. Arkansas Credit Union League Created a simple customer mailing list database for sending out notices, newsletters, and reports. The database appearance is a simple point and click menu driven system. Arkansas IOLTA This agency keeps up with interest earned in attorney’s escrow accounts when they have been paid on retainer. Monthly interest on these accounts is reported on by banks which may or may not charge a service charge. The system I set up allows new attorney accounts to be added easily and the interest paid by the banks to be added with push button ease. All reports were generated with point and click ease. Arkansas Organ Recovery Agency (ARORA) Modified two existing databases so that reports can be created in a variety of ways that makes it simple for staff to do via user friendly report screens. Department of Workforce Education Created a database to track revolving loan clients. Database produces amortization schedules and all loan documents, including approval letters and contract data. Reports are produced easily through menu-driven interfaces. Another database was set up to track equipment available to loan/donate to handicapped people. Equipment is checked in and out as needed. A third database was created to track calls to the agency. Welfare To Work Created a way for all districts to enter the data quickly and easily and for the department staff to get reports with point and click ease. Successful Employment Through Assistive Technology (SEAT) This database involves collecting the data and being able to report easily from the data collected in a timely manner. The database being is easily updated and provide a versatile method of reporting. Technical Assistance/Training Database Created a new training database that fulfills the current reporting requirements. In addition, created a user-friendly Report Menu that will allow selecting date ranges for reports. Telecommunication Access Program (TAP) The Telecommunication Access Program (TAP) was currently in an Access database but new fields needed to be added, reports created and an easy way to generate date-driven reports is needed. Ticket To Work Created a way for all counselors to enter the data quickly and easily and for the department staff to get reports with point and click ease. Arkansas State Bank Department Developed an employee training record database from Unix records. Reports and data entry screens are simple menu driven screens. The database tracks costs by vendor, employee and course taken for any date sensitive time frame. Arkansas State Board of Chiropractic Examiners Developed an database of Chiropractors, tracking continuing education hours, fees, complaints and other information. Reports and data entry screens are simple menu driven screens. AR Women's Business Development Center Converted SPSS data into Microsoft Access. Their program tracks clients through training and business development. Reporting is simple via a Report Menu page. Another database (Ms IDA Extra) was developed to add features that their Ms IDA program did not capture. Reports by grant source and county, as well as demographics were created. ASCO Vending Co Asco is the main contractor providing commissary items for the Pulaski County Detention Center. I wrote a program to enter orders for the inmates to get commissary items. Reports come from a simple menu driven screen. Created a master database that is attached to their Platinum Accounting Btrieve files. Users can then import or attach onto this master file and create and used the data without needing access to their accounting system. This method is also much faster than attaching to Btrieve. Also created an order entry system that ties back to Platinum Accounting as well as a reporting system for bill of materials. In addition, I also created a Document Control database for them to keep track of engineering drawings. I have created several small single user applications for Audio International, such as an Employee Training database as well as the above projects.

Medical Center Created a database to track answers to various questionnaires concerning medical, surgical and social issues. Cardiovascular: Heart Surgery/Transplants/Testing Designed individual department databases and tied them together so that all can share information about their cardiovascular patients. Another database that I created was certified by the American College of Cardiology. Neonatal Intensive Care Unit Pharmacy Created a stand-alone program for the Pharmacy and other departments to easily enter a baby or pediatric patient’s name and weight and get the correct dosing on several drugs. Also created IV dispensing based on the weight and strength of the IV fluids. Pharmacy Re-Orders Created a stand-alone program for the Pharmacy to reorder daily amounts used without over-buying. The program tracks on hand quantities and purchases so that over-buying does not take place. CRP study Created a program to enter patients that are both high and low risk determined by their CRP levels. The study also tracks lifestyle risks such as smoking and what effect these have on CRP levels. Baptist Health Corporate Compliance Department Created several databases to track denials, excessive days that had benefits denied, suspended physicians.
Baptist Medical Center – Arkadelphia Patient Scheduling Developed a patient scheduling database that can be printed by doctor, clinic, patient, and surgery on a daily basis. Once the patient has been entered once, the appointments can easily be added without having to repeat the patient information. Home Delivered Meals Developed a database to track delivery of home delivered meals statewide. LifeLine Developed a database of Life Line patients and their billing. Monthly Reports Developed a database from a QuatroPro file of monthly statistics that can easily be produced with point and click ease.

& West Engineers, Inc. After their old DOS based database crashed, I was able to retrieve the data and create an Access database that allows them to track projects quickly and easily. Beverly Enterprises, Inc. (Ft. Smith) Developed a budget tracking program for their IT department. Big Brothers Big Sisters of Pulaski County, Inc. Created a database to match volunteers with kids. The system keeps track of volunteer information as well as the ‘client’ (the children) information. The system can produce reports using a simple ‘point and click’ interface. Birch Tree Communities Created a database to handle referrals, members (patients) for this organization that houses the severely mentally ill. Other databases for Birch Tree included vehicle maintenance and human resources. Candy Bouquet International, Inc. Worked with staff to develop a simple way to track franchisees so that anyone could run reports and enter information with simple point and click ease. Catering To You Created an easy to use database to track catering events and menus. One button will show all events scheduled on that day; entering a start and end date will show all events between the dates. System includes invoice and delivery sheets. Center Point Energy Created an easy to use database to estimate construction projection, track employee training, special events. Several other databases have been created on a as-needed basis. Century 21 Regional Support Center Created a database from one originally done in Enable to keep up with the training needs in the Arkansas-Louisiana-Texas area. Brokers are required to take so many hours of training per year. If they completed all their training, they got a refund of part of their tuition. The database generated reports of attendees, created name badges for them, and let the Regional Center know who was due a refund. Church Link Created a database that tracks clients and charges and invoicing churches for web services. Christie's Created a database of clients so that labels can be printed when she wants to notify customers of a sale, etc. Clinton Cultural Center - Hot Springs, AR Converted several Macintosh ClarisWorks files into Access database tables. Clevidence Insurance Inc. Created a client database that tracks clients and type of policies they have. Concert Staging Services, Inc. Created a database to make bids on jobs. Jobs involved labor, trucking, taxes, materials, etc. all with point and click ease. Custom Microsystems Created a Service Call database that incorporates customer info, serial numbers, parts lists, and service calls to track incoming calls for on-site service.

Dassault Falcon Jet Planning and Service Center Sales Created a database to keep up with Requests for Quotes (RFQ) on corporate jets. Reports are generated with a simple menu driven system. Pricing estimates are also generated using a simple data entry screen. Another database tracks things to do on incoming jets (squawks). Quality Control Created two databases – one database tracks corrective actions on IS0 9000 requirements and one to track vendors and their PO’s Flight Operations Worked with staff to create a discrepancy-tracking database for use after test flights Davis Process Service, Inc. This company serves papers such as subpoenas and other legal documents. They need to track who has the papers, how may are still out, notifying clients that the papers have been served, etc. Delta Rivers Nature Center The database I wrote for them tracks inventory in their gift store. Invoices and POs are tracked as well as damaged items and donated items. Entergy Arkansas Created an Event Tracking database to tie expenses of various events to their budgets. Vendor and time frames can track vendor expenses with simple point and click ease. Fluid Solutions Created a database that tracks work orders and PO's and Invoices. Glazer's Distributors of Arkansas Created a database that tracks expenses on marketing materials for their vendors and customers.

Advantage/ Created a system called Special Delivery, which keeps up with pregnant women who are covered by HMO Arkansas, Blue Cross Blue Shield, Health Advantage, and others. The database is entirely point and click and includes a nursery rhyme theme for the non-technical users who will be using it. They can track the number of high-risk pregnancies and send the appropriate information to the right person. Letters to physicians are automatically generated at a push of a button. The system is entirely point and click. Also designed a database for Case Management, which tracks patients under case management as well as does the reporting for the Case Management Team by region and line of business. I. K. Electric

tracking system, purchase order systems, small job bidding system,
and IBEW notification system. Job information can be filled out on
screen while the customer was on the phone and printed to give to
the next available electrician. Jobs were tied to PO’s to track
materials. Jefferson County Tax Collector’s Office Worked on site with staff to convert existing database from Rbase into Access. Menus, forms, reports, macros, queries, and other events were set up in an easy to use, menu driven format. John Prince Company Created a database to track bids to contractors. This non-profit agency had an existing Access database but needed help in grouping contributors such as those who had given in the past but were not current contributors. I set up easy to use menus and screens. Lasiter Asphalt Maintenance Co., Inc. Created a job tracking system. Although they only have about 350 jobs per year, each job can have several sub-jobs (paving, striping, excavating, sealing, etc.). The salesman could estimate the job and what sub-jobs were needed and assign them to different foremen. Weekly reports showed which jobs were still open, which were due to start, and which were completed and invoiced. A more recent database (2003) figures production based on man hours and materials used. Little Rock Family Planning After one of the doctors had started a database, I enhanced it and created reports necessary for their business. State reporting is easily done and exported to a file that they can email to the reporting agency. Little Rock Parks and Recreation Department Created a system to keep tract of contract labor payroll. The system allows easy addition of vendors, which are tied to a requisition number. The requisition is printed either from the screen or in batch mode. Reports included list of vendors, dollars paid by organization code, contract payroll by month and year. Created a Project Management system to report progress on ongoing Parks projects. Little Rock Realtors Association, Inc. Wrote a system to bill the association's members Little Rock School District Worked with staff to incorporate Access with their on-line database. On site Access instruction by working one on one with staff members. Maple Leaf Canvas Created a database that would calculate fabric, metal needed, proposals, calculations, shop notes, etc for making custom awnings. The database prints 3 reports at once with a simple click of a button: Contract Proposal, Awning Calculation Sheet, and Job Sheet for internal employees to work from when creating the awnings. Maris, West and Baker, Inc. Developed a comprehensive 800-Call database that could import raw data into it and generate reports at a push of a button. The database is used to track responses to 5 programs that Entergy has set up using a series of several 800 phone numbers. Also created another database to keep up with “Grass Roots” requests. After their programmer moved away, I made many changes to their existing database, which was used to track attendance to corporate meetings. The system creates name badges, check-in lists, hotel assignments and more. Network Services Group Created a service center job log in, an employee database, a prospect database, form letter generator, a customer configuration database, with a simple point and click reports menu. I also go in monthly to append new data into a SWB telemarketing file and have developed reports to this telemarketing database. I wrote a database to track billing and payments for a personal trainer. Also did their website

of Professional Programs – Wrote a program to track continuing legal education hours, report demographics, assign fees. All reporting done with a series of simple fill-in-the-blank criteria and push button ease. A second database tracks Court Reporters continuing education. A third database tracks applicants for the Arkansas Bar Exam. A fourth database tracks Ad Litem attorney training. Powers of Arkansas Created a database to track bids to contractors. Procraft Liquid Siding Company Created a customer/prospect database. The database gives them a way to print a customer referral list with a simple click. It also has a way to print follow-up calls to make and a history of contacts with the customer/prospect so a salesman would have a background to go by. Customer personal fields such as work and home phone numbers, spouse information, as well as hobbies, etc. makes this a useful tool for sales. Pulaski County Humane Society As co–chair of the Dog Wash ‘n Dip event, I created a database to keep up with volunteers, call lists, thank you letters, labels, sign–in sheets, and reports with simple point and click menus. Pulaski Heights United Methodist Church Created an events calendar database to track rooms that were being used each day by which groups and what they needed to set up for each group. Ransom, Dr. John and Kathy After starting a database to track their cattle by herd, their original programmer became unavailable. I re-designed the database and incorporated all the new features they needed to run this side business. John Ransom is a heart surgeon and this is a part-time business run by his wife, Kathy. Raytheon Corporate Jets I converted existing DataEase databases into Access. This database will keep track of some 40,000 drawings such as mechanical or avionics for corporate jets. Also a parts list from drawings is part of the system. I also created a Completion’s Planning database to track bill of materials needed for aircraft being built. In addition to these projects, several other stand alone applications have been built for other departments of Raytheon, such as Pricing and Design. Refrigeration & Electric Company Created a way for them to import their accounting information on customers into a database and get monthly sales, profit totals, and aging of accounts by selected customer. Alumina & Ceramics Lab Developed a lab testing system to track samples from request to completion. Rockefeller Foundation Created an easy to use database to match attorneys with prospective employers. The database creates resumes and allows someone to enter the criteria and out come the resumes that match that criteria. RSVP Catering, Inc. Created an easy to use database to track catering events and menus. One button will show all events scheduled on that day; entering a start and end date will show all events between the dates. St. Vincent’s Worked with staff on a Y2k Access database to customize it for their needs. New Outlook – created a database to track cancer patients and what services were given to them. modifications on an existing Access database Created a complete reporting system to track customer's software licenses. The Computer Training Center Created a student database for them to keep up with their night students enrolled under the American Institute of Banking. They had to report grades, amount charged, drop fees, etc. The system was a simple point and click menu driven one, complete with printing confirmation letters when the student enrolled. Created a complete reporting system on agent sales and listings, commissions, rankings, and buyer/sellers. The system also created their open house ads, multiple listing service data, vendors, suppliers, and Million Dollar Club awards. Created a complete customer claims tracking system that accounts for commissions, premiums, and claims history by policy. The menus are all point and click. Timex Corporation Worked with the Technical Support Staff to develop an Access form to keep up with customer problems and several reports from the data that they collected. Generating reports as well as table maintenance can now be done with a simple point and click Main Menu. Created a system from one originally done in dBase III. It is used to keep up with orders, sales, commissions, and bids. Weekly sales reports by salesman, product lines, bids, and upcoming jobs are done with simple point and click menus. UAMS - Department of Medical Technology Created an easy to use, menu driven database to compute grade point averages by category. Letters are generated automatically to let the applicant know their status and needed requirements. Reports and data entry were done by simple point and click menus. UAMS – College of Pharmacy Created a system to track students enrolled in the College of Pharmacy’s curriculum. All letters and reports are a simple point and click operation; including envelopes and labels. Other databases involved a simple way to grade tests and report the results to the student. UAMS – Arkansas Cancer Research Database to collect mammography data for the year 1997. Unleashed Innovations, Inc. Set up a database to track guests at their three vacation homes in Gatlinburg, Tennessee. Thank you notes are generated, as well as envelopes, reports, and labels. United Transportation Union Created a database to track union claims. West Tree Service Created a database to track employee issues such as training, HR and other area such as accidents, etc. Williams & Anderson, PA Created a database to track client matters. Originally a product of two separate WordPerfect databases, I converted their data into one database. Simply menu-driven controls make the data available to all employees. Wittenberg, Delony, & Davidson Took an existing dBase file and created a comprehensive Project Tracking system that is entirely menu driven. Reports and data entry is done using easy point and click options on every screen. Another database of clients was also created using a menu driven interface. A third database tracks fees. XL Connect Created a software purchase logging system to keep up with the software that was bought by their large customers. Reports included a confirmation letter back to the Company, as well as who ordered it, the cost center, etc. Also, I created a database to input training classes and students enrolled into them. It prints class rosters, certificates, and confirmation letters with point and click ease. Zeneca Ag Products (North Little Rock) Created a series of databases to connect the various departments; from receiving to the lab to shipping – tracking the products active ingredients from the manufacturer and comparing it to their own lab’s results.
